Psychotic depression: Some depressions have psychotic features, including delusional thoughts like one's spouse isn't trustworthy. This is a more complicated form of depression than the usual variety, and may need antipsychotic medication along with antidepressants. If the person has been suspicious all along, this may also be an accentuation of ongoing views of life. Not pleasant for spouses or others, i'm sure.
