ID, records: New patients should bring with them on their first visit: - valid picture id with current information - health insurance card - co-payment - list of medications - any radiology reports (ultrasound, ct scan, mri, mammogram, etc.) - any test results (biopsies, lab reports, etc.) and physician notes pertaining to the present problem you will be evaluated for.

W What kind of appt?: What kind of appointment? Please ask your question again, and clarify. Before most first medical appointments, it's a good idea to bring all your medicines, including prescription, over the counter and supplements. Also bring any records from other doctors that you may have.
