Hernia: Painful lump at or around the umbilicus is usually a hernia, especially if it gets worse with coughing, lifting, straining... Easy to fix but he should get checked to be safe.

Umbilical hernia: Get a surgical opinion, might need a ct scan to elucidate the source of pain.
