Enuresis: Ammonia smell can be normal but you put these 2 together and i get really concerned. This needs further investigation by a primary care doctor first who may refer the patient to a children's kidney specialist such as myself.

Diet? : A couple of reasons come to mind. Either she is eating a lot of protein or she is not getting enough fluids. If this persists talk to your doctor.

Maybe an infection: You should take her to the pediatrician to make sure she doesn't have an urinary tract infection.
