Mood/depression: Depression is associated with increases irritability and poor self esteem, the later leads to feelings if being less and being targeted...Some people direct the anger resulting from irritability inwards, meaning they hurt themselves. . This is treatable, find a psychiatrist to do the evaluation then he should be able to prescribe the right medicine and connect u with the right therapist.

Need eval to know.: Sounds like your moods are not well regulated & that you lack healthy coping skills. Please see a psychologist or a psychiatrist for a comprehensive assessment and to begin treatment such as dialectical behavioral therapy. Take care.

Good eval needed: Borderline personality Disorder comes to mind , it is also likely that you have a sensory processing disorder-which means you need to put a fair amount of work in to keep yourself regulated, getting into a good counseling relationship will be important, working on cognitive strategies and developing routines to keep you on track will be important -thins includes diet, exercise, positive thinking.
