Why is there a need (rationale) to separate the serum from the whole blood immediately? any changes that might occur if separation is done immediately

Yes: If the serum is left in the same tube with the red blood cells, components of the serum will undergo changes that will therefore give erroneous results. For example, glucose will be reduced, while potassium slowly rises.
