Distress or conflict: Between people is a relationship difficulty. Most of the time, people work things out by themselves. Problems: The most obvious answer is it means problems in your relationship. This may mean a lot of conflicts, general unhappiness with each other, poor communication, etc. Not sure what your looking for but difficulties are subjective .. It's a relationship problem if you think you have a problem.
