Ankle instability: Likely have a stretched out or torn ankle ligament. This can be treated via surgery if that is indeed the problem.

Ankle gives out: The most common cause is an old, chronic tear of the supporting ligaments on the outside of the ankle ATF and CF ligaments. PT may help give strength to the supporting ankle tendons, but if the ligaments are lax or torn, they will need to be repaired.
