Inflammation: The redness is usually just inflammation caused by the cold sore virus. Rarely it is a secondary bacterial infection.

See below: Inflammation. Which can be simply due to the cold sore or other issues like a secondary bacterial infection. However, if just started, it is likely just part of the inflammatory process with the cold sore.
