Cables or Dumbell: I would recommend you consider doing all overhead presses and pulldowns in front of you rather than behind. Also try and keep a firm neutral spine posture to prevent secondary spine issues. Dumbells allow greater freedom of motion and allow you to build some secondary stabilizers. Using isolateral cables may also let you achieve similar results.
