no short answer: Is the mitral regurgitation (mr) mild, moderate, or severe. 45% ef is mild hypokinesis in general. But if the mr is severe, 45% includes a large amount of blood going backwards, and much less going forward. Consider possible surgery in that case. If the mr is mild treat with medication. Asa can stand for different things. What does it stand for in you?

Mitral reguritation: You need a complete cardiac evaluation with a consideration toward corrective mitral valve surgery, preferably repair rather than replacement, at a reputable cardiac center that has recognized experts in this type of surgery. By asa, are you referring to aspirin?

Not normal: You need to know the severity of mr. If it is severe and your ejection fraction is low, you likely need mitral valve surgery. Sometimes, hypokinesis can affect the mitral valve function by "tethering" the chords supporting the valve. This can maybe be helped if is due to blockage to the heart artery which can be fixed, and function improves.Trial septal aneurysm does not affect mr or hypokinesis.
