Genetics: There are 2 main reasons why ivf can fail. One is a problem with the uterus. A fibroid, polyp, scar tissue, or an inadequate thickening of the uterine lining. Assuming that is not the case, the most likely reason is a genetically abnormal egg. At age 39, almost half of all eggs are genetically abnormal. When that occurs and a pregnancy implants there is a 90% chance of a miscarriage.

Less healthy embryo: So sorry for you. Your quants aren't encouraging. It's hard to say for sure but the embryo "quality" is very important in their survival. Your age may be a factor :-( as we age our remaining eggs are less "healthy", and your husbands sperm may have an impact, though i don't know all the details. Even ivf with the best doctors can't guarantee a baby. See your doctor and god bless.
