Yes: A femoral hernia is an inguinal hernia through the femoral canal behind the ligament which will make it difficult to identify by physical exam. Moreover acute small inguinal hernia is usually associated with pain due to its acuteness and us difficult to identify by physical exam. I hope this answered you question.

Yes: It is possible. Evaluation by surgeon would b the best option. If too small to feel on exam, ct or MRI can b helpful.
