No: Chronic cough will not cause a hernia but it will worsen it. From your question, it seems that your son have multiple pulmonary problems that he was born with at 24 wks and hiatal hernia was one of them. Yes: Chronic coughing would cause increased abdominal pressures. That increase can contribute to a hiatal hernia.
