Probably not: Heart transplant requires heart/lung bypass for an extended period of time while the surgery is performed. There's limited data available but maternal mortality is as high as 25%, to say nothing of the baby. The ultra-high dose immunosuppression used for induction would be pretty bad for baby too. In short - no.

No: When pregnant it can be dangerous to have any surgery. But a heart transplant requires being placed on the heart lung machine and being on very high dose of blood thinners. I don't think the baby would survive the surgery.
