At age 91 . . .: Sleeping a lot is reasonably normal, since there isn't a lot of physical or mental activities that are possible or interesting, depending on each individual. Activity is good, but so is sleep. If you are concerned about a possible medical condition, have her seen by her family physician, but don't go crazy with tests and evaluations--she's 91 and entitled to not have everything in "perfect" shape!

Probably yes to both: As one nears the end of life, everything slowly slows down. Exercise and a healthy diet are good and would let her guide you with how much is needed. Have her see her doctor to see if something relatively simple might be the problem (thyroid, anemia from low B12 or iron), but anything more serious will likely cause more harm to evaluate and/or treat.
