COPD?....: It is possible that your father has copd, chronic obstructive pulmonary disease as a result of his heavy smoking. Pulmonary function testing would confirm this diagnosis. There is no cure but there are treatments available. Smoking cessation is absolutely needed to decrease airway inflammation and slow disease progression. Your father needs to see his doctor for diagnosis and treatment.

Lung scar: Parenchymal scars are usually the result of old infection or trauma to the lung tissue. Oftentimes, these scars can become cancerous.

Workup and Mgt: There are several possibilities. If the scarring is localized to the top regions of the lung only, and there is no notable air trapping, it is possible that his lung function is preserved. I would recommend a pulmonary function test with and without a bronchodilator and avoiding all smoke exposure. If he is producing sputum, then a culture and cytology may be ordered as well.
