Depend overall healt: It depends on your overall health. It should be a short procedure between 30-45 min and if conservative measures have not helped in over 6 months and it is interfering with quality of life and causing moderate discomfort, it makes sense to proceed with surgery. If you do not have co-morbid issues that can cause problems with surgery (liver issues, poorly controlled htn, diabetes), emphysema ok.
