ER : With an inr that high and not feeling well, you need to take him to the er to be evaluated now. Good luck.

Yes.: He is at risk of serious bleeding. He should be treated with vitamin k and have his urine and stool checked for blood.

Without bleed: Usually can stay at home hold coumadin, (warfarin) obviously see office doc once open treatment if > 9.
