Need Imaging Study: You may need to rule out any process occurring at the spinal level, such as a herniated disc. These are less clear to diagnose if they are at the thoracic level. An MRI of the thoracic and lumbar spine might be useful if discussed and approved by you family physician.

Shingles: Shingles can present with severe pain without a rash. The pain is usually in a strip of skin called a dermatome. Neurontin (gabapentin) may be beneficial. I would discuss treatment of shingles with your doctor. Usually the pain slowly improves over time. Good luck!
