???: There is no one main disease paradigm ... Each disease/problem has to be addressed and a paradigm of diagnostic and treatment options are then developed and followed systematically.

Differential diagnos: physicians get trained in diagnosing and treating diseases and illnesses, it is a very long process and is always ongoing, therefore there is no single one answer, I hope this helps
