You should treat: While the symptoms of scarlet fever will improve on their own after 2 weeks, you should treat with antibiotics. 2-4% of people with untreated strep infections of this type can get heart damage (known as rheumatic heart disease). It doesn't seem like much, but i wouldn't risk my heart.
