Primary ESS: Primary empty sella syndrome is the result of herniation of the arachnoid into the sella tursica causing the pituitary gland to be compressed against the floor of the sella and enlargement of the bone, resulting in an 'empty' appearance on imaging. It is more common in women and the most common symptom is headache.
