Is it the mom or dad's fault when it comes to abo incompatibility?

No: Assigning "fault" or "blame" is useless in this situation as there is literally nothing either can do to prevent ABO incompatibility.

Nature's: Only if mom is o type blood you get ABO incompatibility, at the same time dad's blood type has to be a, b or ab.
