A : A "whooshing" sound that varies with positional changes of the head and neck may represent turbulent blood flow through a partially narrowed carotid artery system whose internal branch runs close to the middle ear internally. Other entities, such as glomus tumors potentially could present this way. Tinnitis, often caused by high frquency nerve deafness, typically isn't positionally related. I would start by seeing an ENT physician to rule out otologic causes first, have someone listen to your carotid and vertebral arteries for bruits, and consider duplex studies of your vessels if deemed appropriate.

If : If the sound is machine like it is likely vascular in nature. It could be caused by carotid artery in which case one needs a vascular workup. Additionally increased intracranial pressure may also cause a swooshing sound in the ears we call it venous hum. In which case an eye exam and MRI may be needed. A good start though is a hearing test and a good history and physical exam.
