Cardiac pain can present in many different ways, but is often beneath the sternum (substernal), can radiate to the neck or arms, and is often associated with palpitations, sweating, and nausea. Pain from the aorta, the large blood vessel in the chest, can send pain to the back, and can cause blood pressure discrepancies in the arms. Pain from the esophagus or from the lungs can cause chest pain. Muscle pain can also cause chest pain, often for no discernable reason. If a patient is having new chest pain with other symptoms, then they should seek out a medical evaluation.

Yes: Er eval is a good idea. You have chest pain and left arm symptoms radiating from the neck, these could represent angina, which means an impaired blood supply to the heart muscle from the coronary arteries. There are certainly other causes of chest and arm pain, but your combination of symptoms sounds concerning. It's better to be sure, go to the er.
