Prurigo Nodularis: The scar was formed from repeated scratching. Sometimes injecting a potent cortisone preparation (kenalog (triamcinolone) being most commonly used) may reduce the size. If this didn't work, then you may consult a dermatologist or plastic surgeon to get it removed. The condition is known as prurigo nodularis.
