Fat : Fat grafting to the eye area is a great way to address sagging and a sunken appearance. Improving the volume in the region helps provide more normal support and lift to the skin. Without seeing you it's hard to say whether or not blepharoplasty would also be helpful. It is possible to have both surgery depending on your anatomy.

Fat transfer eyelids: Without a photo or an exam it is impossible to tell, however fat transfer for sunken lids are very helpful in many and a blepharoplasty can remove redundant skin and redistribute the underlying fat if needed. Peels and lasers can improve the skin texture and eliminate fine wrinkles.

Seek a consultation: You've hit on a very important question. It is impossible to give you a detailed answer without seeing your eyes. Fat transfer can be highly variable. The appearance of the eyes can typically be dramatically improved with surgery. The question that a consultation will answer for you is exactly what procedure(s) will be most helpful.

Possibly: if you have saggy eyes with fat pads then fat transfer is a great option. Ask your facial plastic surgeon.
