A member asked:

When would you deliver? if you had a patient that had ruptured membranes at 29 weeks and she went into labor what would you do if after keeping her pregnant for a week she started labor and you checked her and noticed a blood clot about the size of a half

There : There are many factors that would enter into this decision. The health of the mother and the fetus, such as is the fetus in any distress, would enter into this decision. In general, delivery would be delayed at 29 weeks as it so dangerously early. The fetus's lungs are not developed. There are steroids that can be given to try to develop the lungs earlier. It would be important to know if the mom was having problems, such as a fever, or other serious medical problems also.
