I have pain in my left breast and don't know why it is painful and very tender i feel it hardening in one particular spot it's not muscular or caridio problem the er refuses to help me they just sent me home i also have had nipple discharge but it's clear

Please : Please be seen immediately by a gynecologist or breast surgeon for this. You need a complete and thorough investigation. Any pain and hardening in the breast must be taken very seriously. I agree with your concern. Please be seen immediately for this concerning complaint.

Breast specialist: You should see your primary care or gynecologist who can refer you to a breast specialist. This is not a problem that can be managed in the emergency room. Look up fibrocystic changes in some of my other posts, it will help prepare you for your doctors visit.
