Since : Since the abdominal pain is very severe and tender to the touch, i recommend that you go to the emergency room. The er doctor will need to palpate your abdomen to see if you have signs of a "surgical abdomen". Right lower quadrant pain is often associated with appendicitis.

In : In the left lower quadrant, the differential diagnosis includes: diverticulitis, ovarian cyst or other pathology, pelvic inflammatory disease, kidney stones or urinary tract infection. On occasion, the pain can be from appendicitis, pancreatitis or peptic ulcer disease. Other symptoms, such as fever, diarrhea, burning during urination, vaginal discharge, nature of pain (moving, crampy, referred to another location) can also assist in narrowing down the causes, but given the severity and point tenderness, suggesting a localized process with peritoneal signs, you should head into your physician for testing, including a ct scan and bloodwork.

With : With the limited information you have given, it is safest to be seen by a medical professional. An urgent care center or emergency room would be a good start if you are concerned that something significant is occuring.

If : If your pain is as severe as you describe, you should see a doctor very soon. An urgent care center or emergency room may be indicated. Also maybe you family doctor could see you for an urgent problem. Sometimes diverticulitis can cause what you are describing. Sometimes there could be ovarian cyst condition or urinary bladder infection. You should have an examination very soon.
