Yes: It is very rare, but left untreated a large PDA can lead to heart failure and death.

No fatalities: A PDA -if large- can be fatal because it can lead to pulmonary hypertension. If diagnosed and treated early it leaves no residues. Pts have a normal quality of life and life expectance.

Possible but....: It is possible that a large pda, left untreated, could lead to fatal congestive heart failure. But this is uncommon, unless you have no access to medical care. Pdas are not associated with sudden death and they can be tolerated for a long time.
