You : You have been dealing with some serious issues. It seems as though everything under the sun has been prescribed and failed. If your infection has spread to the bone then oral antibiotics will not work. You may need to be hospitalized for IV (intravenous) antibiotics if your dentist is unable to remove all of the infected bone. All the best.

Seek care of ENT: You should go to an ENT to secure a differential diagnosis. They are better prepared for sinus infections. If they do not find anything and suggest it is oral in nature, contact a dental school and ask for their help.

Tough one: Oral antibiotics don't work for all problems. Need to find source of infection and eliminate it. Would recommend seeing both endodontist and ENT specialist, and ask them to consult w each other.
