Surgery : Surgery is best reserved for mechanical issues such as cartilage/meniscus tears, loose bodies, or ligament reconstruction/repair. To determine whether these issues apply to you, often an MRI is needed. If you are diagnosed only with arthritis/wear and tear, arthroscopic surgery is less reliable as a solution and you may be better off trying more conservative treatments such as injections, exercise/therapy, activity modification, bracing, etc.

Depends: X-rays alone cannot always diagnose a problem. I would not jump into any surgery unless the surgeon has a good sense of the pathology or problem. An MRI or ct scan might give more information to the surgeon and to you for good decision-making.
