Pinched nerve: Disc herniation in neck can give neck pain and shoulder pain. The shoulder pain is a manifestation of a pinched nerve.

Nerve compression : Most commonly its a muscle strain. But it can also be a symptom of nerve compression in the neck that radiates to the shoulder. This is worth getting checked out.
