SIRS: Sirs results from the perforated bowel. The addition of the bacteria from the bowel spilling into the abdomen greatly increases the bodys response to this critical condition. This increase in immune function can push a patient into ards.

Inflammation: Whenever one sustains an injury or severe illness the body respond with inflammation. This is part of fighting infection and healing injury but also contributes to the damage when the response is severe. When the bowel is perforated large numbers of bacteria leak into the abdomen causing very severe inflammation that involves the whole body. The lung then gets inflamed and fluid filled ( ards).

Sepsis: Perforated bowel may result in severe infection which can spread to blood stream and trigger severe immune response (to fight the infection): sepsis. This will result in the release a lot of chemicals that can cause damage to the lung tissue leading to ards.
