See a podiatrist: You could have an infection, you could be delivering a blister from shoe irritation, you could have arthritis. Best to see a podiatriast for evaluation.

Some possibilities: Bursitis, gout, infection, corn or wart are some. Is there swelling, redness/discoloration or wound. Does it only hurt when applying pressure or does it hurt all the time. See podiatrist for evaluation and possibly bx-rays.
