Yes: Sometimes, the pacemaker is not sufficient to prevent other types of arrhythmia. For example, if one gets venytricular fibrillation, external pacemaker would not help.

Yes: Ventricular tachycardia, or fibrillation, or full arrest can still happen. The external or internal pacemaker will continue to fire but is ineffective.
