There are number of different causes of chronic neck pain from musculoskeletal tension, poor posture to other more serious disorders. In any event, if you have not been evaluated by your primary care physician or an appropriate specialist that would be a first step.

Arthritis: Sounds like degenerative joints in neck. Since its been present for a while, i would get cervical MRI and talk to a pain doctor.

Need an orthopedist: This problem needs to evaluated by a medical specialist. Firstly you will need some neck x-rays or an MRI to pin point cause of your pain. You are very young to have such a persistent problem. Then various treatment options must be considered. These include physical therapy, massage, neck support (collar), possibly surgery and finally pain therapy. Good luck.
