See your doctor: This is one of those problems where a visit to your doctor is necessary to figure out what's going on. Only after a thorough evaluation, including examination and possibly labs and other tests, can your doctor correctly diagnose you and treat you effectively.

Dressing and splint: If you faced with problem some were and there no emergency facility close, what you could do, apply sterile dressing if you can and try to immobilize the extremity with some kind splint to help reduce the pain and stop the bleeding. If the bleeding did was a lot apply tourniquet above the bleeding site tell the medical help arrive.
