An M spike in either: The urine or the serum reflects the production of a single (monoclonal) globulin. It is found in multiple myeloma, a malignancy of the plasma cells that make antibody, and also can be found in so-called monoclonal gammopathy of unclear significance (mgus) which is generally benign.
