Frequently: Probably 20-50 percent of subtle lisfranc fractures are missed in the er. Significant swelling, bruising on the bottom of the foot and pain lasting more than 2-3 weeks are indications to seek evaluation from an orthopedic surgeon trained in foot and ankle.

Rarely: It can be missed initially in the er, but an followup xray with an orthopedist should differentiate. Most lf fractures produce a lot of swelling.
