See long answer: 1. Liver irreversibly damaged by disease, or carrying a cancer that not treatable with conventional surgery. 2. Ability to withstand major surgery (basically, heart and lungs ok). 3. Ability to care for organ after transplantation (able to take antirejection meds, no ongoing alcohol or drug addiction) 4. Free of cancer (except possibly liver cancer) and infections.

Liver transplant: Patients must be free of malignancy and infections, be able to tolerate a large operation from a cardiopulmonary perspective and they must also demonstrate compliance, particularly for etoh abstinence. Prior upper abdominal surgeries, obesity, portal vein clots and vascular disease make the operation riskier, but are not necessarily contraindications. Ultimately, the transplant team will decide.
