Several things: It could be a rotator cuff issue, a tendonitis (inflammation of the tendon), bursitis (inflammation of bursa between the tendons), or something called frozen shoulder which is a painful condition where the shoulder literally feels frozen. You should probably seek out a sports medicine or orthopedic specialist to look at it.
