How is a torn esophagus treated?

Emergently: In most cases a torn esophagus is an emergency (if perforated) b/c of the connection to the mediastinum where the heart and other important structures live. Concern is for bacteria or stomach contents to enter into that space and causing inflammatory reactions and infection. Thoracic and in some cases ENT surgeon would be involved in treatment.

Urgent Surgery: Esophageal rupture from excessive vomiting or perforation from endoscopy are treated with urgent surgical repair in almost all cases. The longer it takes to diagnose and get to surgery, the higher the mortality. The esophagus is repaired with suture and usually wrapped with surrounding tissue or muscle to reinforce, then no food for 4-5 days until a study confirms repair. Cancer may change plan.

Rapidly : This requires immediate surgery from what ever the cause. Even treated fast & in the best of hands there is a high fatality rate due 2 rapid development of mediastinitis, which spreads rapidly 2 involve vital contents of the chest cavity. Also septicemia is common with it due 2 vascularity of the mediastinum.
