If at work, you have: To file americans disability act papers with your state's vocational rehabilitation agency & apply for SSI & medicaid. Your hr needs all documents that confirm your degree of disability & need for supports. Time off for medical/ therapy care requires you & your dr.Fill out fmla forms. If you knew about, but didn't disclose your disability on your job application, you may be out of luck.
