Many possible causes: Your pain could be as a result of many different things. A thorough history and physical exam and possibly some laboratory and radiology exams are needed to determine the exact cause. Causes of pain may include: trauma (fracture, sprain, arthritis), circulatory, musculoskeletal or biomechanical abnormality and neurologic conditions (neuropathy).

Depends: Unlike dr. Rafat who is just reposting the same answer to multiple questions i will try a brief summary. Typically your pain is muscular in origin. It could be related to shin splints (posterior medial tibial stress syndrome) if pain is medially and occurs during mid run. It could be vascular but at 43 unlikely. You could also have a stress fx. I would see your doc if pain continues or rest.

Leg pain: The differential diagnosis as to what is causing leg pain with sprints is long and requires more information from you regarding the quality of the pain, the location, severity, and what makes it better or worse, as well as an examination and thorough history taking. Start with your PMD and he/she can send you to the appropriate specialist(s(s). Compartment syndrome is on the bottom of the list.
