Need to wait: There is surgery to reduce a broken nose, but a 4-year-old's nose is not done maturing. Surgery may be needed now to straighten the break, with a possible rhinoplasty once the nose is done growing.

Maybe: Some nasal fractures are treated with surgery and some do not require surgery. The only way to tell would be to have an evaluation to assess the degree of the fracture.
