2 substances: Methadone and tylenol#1 are both controlled substances and pain killer, and together increases the chance of side effects. I am sure that your pharmacist also wanted to make sure about possibility of abuse.

Narcotics: Because tylenol#1 has mild amount of Codeine which is a narcotic ( opioid). Methadone is a stronger opioid. There is lot of abuse these days and due to restrictions, the pharmacists won't give you both unless your doctor clears. Also there is a risk of overdose if you take both.
