Naked eye: A papillary excrescence is a gross finding, meaning that it certainly can be seen by the naked eye. However, what that excrescence actually represents, from a more typical benign papillary cystadenoma to borderline tumor to papillary serous cystadenocarcinoma requires a pathologist to make that diagnosis by looking at the tissue under the microscope.

Both: The general appearance of a cyst can be identified by a doctor but generally this is only truly confirmed and its characteristics are identified by a pathologist.

Maybe both: It depends where that description is from, but it sounds like a gross description (not microscopic) so they are likely seen with the naked eye. Though if they are papillary to the naked eye, they will usually be under the scope too.
